چکیده

Although making a decision for the need for surgical correction of diplopia in blow-out fracture is difficult, it is logical to assume that an effective approach for management of diplopia in this type of injury should consider more than one factor. These factors include a preoperative BSV score and of the level of tissue herniation. Recently, the authors published two papers based on 10 year retrospective data on the influence of both preoperative BSV score and orbital tissue herniation on diplopia outcome. It is possible, on the basis of these studies data, to provide a figure about the potential benefit of surgery. Surgical intervention would be more beneficial for cases with orbital muscle entrapment / muscle herniation with preoperative BSV score ≤60%. Conservative management might be considered for cases with BSV ≥80%. In the 60-80% BSV range, the level of tissue herniation may advise the need for surgical intervention.

Blow out fracture A Novel Management Modality

Blow out fracture of orbit involves fracture of orbital floor without fracture of infraorbital rim. This injury is common from frontal blow to orbit. Frontal blow to orbit causes increased intraorbital tension causing fracture of floor of the orbit (weak point) with prolapse of orbital content into the maxillary sinus cavity. This causes enophthalmos and diplopia. Infraorbital rim is not involv...

Orbital blow-out fractures in children: characterization and surgical outcome.

BACKGROUND Trapdoor-type orbital fractures usually associated with marked motility restriction are common in the pediatric age group. We reviewed the characterization and surgical outcomes of orbital blow-out fracture in children. METHOD This is a retrospective review study. From Jan. 1997 to Dec. 2006, 75 patients under 18 years of age with orbital blow-out fractures were seen in the departm...

Another look at blow-out fractures of the orbit.

Thirty cases of orbital floor blow-out fractures proven by tomography were retrospectively reviewed to determine the accuracy of the plain films. Using the maximum diameter from the tomograms, the area and volume of the fractures were calculated and correlated with the presence of diplopia and enophthalmos at the time of presentation and at subsequent follow-up. Nine of the 30 cases underwent s...

Analysis of complications after surgical repair of orbital fractures.

BACKGROUND The term "orbital blow-out fracture" is referred to as the mechanism by which an impact to the eyeball is transposed as a mechanical energy to the orbital walls, causing them to fracture. Despite a proper surgical technique, a successful anatomic reconstruction of the orbit, and an accurate follow-up, 3 complications are still frequently observed at long-term follow-up: diplopia, eno...

The predictive factors of diplopia and extraocular movement limitations in isolated pure blow-out fracture

PURPOSE To evaluate the predictive factors for development of diplopia and extraocular muscle movement (EOM) limitations in the patients with isolated pure blow-out fracture. METHODS One hundred thirty-two patients with isolated pure blow-out fracture were included. The diagnosis was done with computed tomography scan. Possible predictive factors were analyzed with logistic regression. The ca...
